Medicaid

We are a Texas-certified, licensed provider for Medicaid which is currently paying a 100% full hearing benefit.

If you are currently enrolled in Medicaid, you are eligible to receive 2 hearing aids at no cost every 5 years.

In order to process your Medicaid claim, please bring the following items with you to your appointment:

• A copy of your current Medicaid ID card
• A signed Physician’s Exam Report (Form B.41) Click here to print form.
